Early on National Energy Board had recognized that self inspection by contractors was not the sort of thing that made pipelines safe.
Codes of construction have existed for many years, the hard lessons written in blood. Still business ignored hard facts and government had to create oversight, Regulations... Law. Under the category of law, there were rules about self certification and of course contractors did not want to pay extra for repairs or scrapped materials. The self certification influence was maintained and Non-Destructive Examination was the scapegoat for sloppy quality.
My descent into business crime was when I took of the Non Destructive Examination portfolio and was informed TCPL was in serious non-compliance with three Court Orders prohibiting entrenched practices. The long stories of preceding engineers feeling guilty did not sit well with me. Knowing I was responsible, I started to make changes....
BROKEN TRUST was always about crime by lawyers, and engineers playing lackeys, long before I got to TCPL. It just happened to escalate when I was hired with a new business model, business would make engineering decisions.
Due to the actions of Joe Pavigalanti Professional Engineer, the problem carried on unabated. The false audits only made things worse. Now the public will pay a long term priced for acceptability of unskilled practice as the societal norm. I use aqueducts as both a metaphor and reality as the problems of monopoly infrastructure have never changed.
